2024 Enrolment

As the legal parent or guardian, I release and hold harmless SONIC Performance & Production, its owners and operators from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises or any premises under the control and supervision of SONIC Performance & Production, its owners and operators or in route to or from any of said premises.

The undersigned gives permission to SONIC Performance & Production, its owners and operators to seek medical treatment for the participant in the event they are not able to reach a parent or guardian. I hereby declare any physical/mental problems, restrictions, or condition and/or declare the participant to be in good physical and mental health. I request that our doctor/physician be called and that my child be transported to the hospital listed below.

Tuition is due in the first 2 weeks of each term. If accounts are paid after week 2 there will be a $15.00 late fee applied to the account balance. All fees are payable in advance and are non-refundable. No credit is given for missed lessons however missed lessons can be made up if a similar class has vacancies available. Fees are billed in 4 equal increments of 10 weeks regardless of how many weeks in the term (some terms are 9 weeks, some terms are 11 weeks). In the event where your overdue account is referred to a collection agency and/or law firm, you will be liable for all costs which would be incurred as if the debt is collected in full, including legal demand costs". Lessons will be terminated for those students that have fees past overdue.

I understand that SONIC Performance & Production sends out all correspondence via email and updates on Facebook and the SONIC Performance & Production website and it is my responsibility to read and take note of any important dates and events. All notification will also be placed on notice boards inside the SONIC Performance & Production premises and it is my responsibility to read.

I understand that it is my responsibility to make sure the child participant is picked up and dropped off from inside the venue to ensure the safety of the child participant. I hereby agree that my child participant/s must wait inside the venue for myself and/or an authorised person to arrive to pick them up. I agree that no child participant will be allowed to exit without the presence of their Parents or authorised persons.

SONIC Performance & Production can take photos/videos that may be used in print and/or electronically for any lawful purpose, including but not exclusive to, e.g. such purposes as publicity, illustration, advertising and Web content.

I understand that SONIC Performance & Production reserves the right of admission.
